Insurance and
Payment Information

Payment is
ultimately the responsibility of our clients. In
the rapidly changing world
of Medicare, Medicaid and private
insurance, our goal is to enable you to utilize your
benefits
as fully and fairly as possible. In order to
process any claim, a prescription is required. But
please understand that not all medical supplies and
equipment are covered by
insurance, even if your
doctor gives you a prescription.

A&C Medical
participates in the Medicare and CT Medicaid programs.

Since we are
an out-of-network provider with private carriers, we will
gladly submit your claim
once full payment has been
received, so long as we have been provided a current
prescription
and insurance information in a timely manner.
Any reimbursement will be mailed directly to
you by your
carrier.

For Worker's
Compensation claims, we will take assignment for charges
over $100.00, with a
signed approval of the manager of your
claim.

Under certain
conditions and diagnosis, Medicare and some insurance plans
cover diabetic
shoes and inserts.

Medicare pays
80% of its "allowed amount". The patient is
responsible for the other 20%
(for many patients this is
covered by their secondary insurance carrier). For

No persons
can guarantee payment by any insurance company.
Reimbursement is
always at the discretion of your carrier,
in accordance with the terms and limitations
of your plan.
